Your initial evaluation is $175 self-pay. It covers the visit only — it decides if you're a candidate and what will help. Labs and medication are separate.

Insurance:

Certain PPO plans (insurance that lets you see out-of-network doctors) may apply to your first office visit; you'd pay your copay or deductible. After that, follow-ups are membership or self-pay.

Labs are covered by any insurance,

whatever your plan — even if our office doesn't take your insurance, the lab does. Without insurance, labs run about $91 minimum.
