Low Testosterone Symptoms in Men

Low testosterone symptoms fall into three groups: sexual (low sex drive, weak erections, infertility), physical (fatigue, muscle loss, more body fat), and mental (low mood, irritability, trouble focusing). Sexual symptoms are the strongest sign of low T — the physical and mental ones overlap with aging, stress, and poor sleep, so a blood test is the only way to confirm low testosterone.

Sexual symptoms of low testosterone

The sexual signs of low testosterone are low sex drive, erectile dysfunction, fewer morning erections, and reduced fertility. These are the symptoms most tied to low T, because testosterone drives every stage of the male sexual response. A drop in sex drive is the most common and most telling sign.

Low sex drive (libido). A clear drop in interest in sex.

Erectile dysfunction. Trouble getting or keeping an erection. Low T is one cause among several, so ED alone doesn't confirm it.

Fewer morning erections. Fewer erections during sleep, in the morning, or that happen on their own.

Infertility or low sperm count. Testosterone supports sperm production, so low levels can make it harder to father a child.

Physical symptoms of low testosterone

The physical signs of low testosterone include fatigue, loss of muscle and strength, more body fat (especially belly fat), breast tissue growth, less body hair, and weaker bones.

Fatigue and low energy. Feeling drained even after a full night's sleep.

Loss of muscle mass and strength. Muscle gets harder to build and keep.

More body fat, especially belly fat. You gain fat and lose muscle.

Breast tissue growth (gynecomastia). Swollen or tender chest tissue.

Less body and facial hair. Slower growth or thinning.

Hot flashes. Sudden warmth and sweating, like menopause — usually only with very low levels.

Weaker bones. Over time, low T reduces bone strength and raises the risk of fractures.

Mental and mood symptoms of low testosterone

Low testosterone can cause low or depressed mood, irritability, trouble focusing ("brain fog"), and low motivation. Testosterone affects mood and thinking, so these signs are real — but they overlap heavily with depression and stress, so these low t symptoms rarely confirm low T on their own.

Low or depressed mood. Persistent low spirits or loss of interest.

Irritability. Shorter temper than usual.

Trouble focusing ("brain fog"). Concentration and memory feel off.

Low motivation and drive. A general drop in get-up-and-go.

What are the symptoms of low testosterone most likely to point to low T?

Sexual symptoms — low sex drive, weak erections, fewer morning erections — are the strongest signs of low t, meaning they're more likely caused by low T than by anything else. Physical and mental symptoms like fatigue, weight gain, and low mood are weaker signs: they happen with low T, but also with stress or poor sleep, so on their own they don't tell you much.

That's why doctors don't diagnose low T from symptoms alone.

Do low testosterone symptoms differ by age?

Somewhat. Testosterone declines slowly with age, so symptoms appear gradually and are easily mistaken for normal aging. Younger men usually notice sexual symptoms and fatigue first, while muscle, bone, and body-fat changes build over years. Age doesn't change whether a symptom counts — it changes how easily it's missed.

These symptoms overlap with other conditions

Most low testosterone symptoms aren't unique to low T. Fatigue, weight gain, low mood, and poor focus also come from poor sleep, stress, depression, thyroid problems, and sleep apnea. This is why a symptom checklist can't diagnose low T — only a morning blood test confirms it.

Being overweight can both cause low T and produce the same symptoms on its own. If your levels come back normal, the cause of your symptoms is something else worth chasing down.
